Case 1


  • Name: TG
    DOB: 9 y.o. / M
    Tanı: Behcet Hastalığı + DVT
    Yukardaki problemler nedeniyle hastaneya yatırılan hasta, yaklaşık 30 gündür tedavi
    görmekteymiş. Hasta birkaç girişim sonrası stabil hale gelmiş. Hastanın ailesi gurbetçi işçi
    olduğundan, tedavilerine yaşadıkları ülkede devam etmek istemişler ve çocuklarının
    yurtdışına sevk edilmesine karar vermişler.
    Transport Report
    Hasta XX trrihinde görüldü. Muayenede; yorgun görünüyordu. Bacakları şiş idi. Kısa
    mesafeleri yürüyebilmesine rağmen, yürürken ağrıları oluyor ve zorlanıyordu.
    20 gün önce, sağ popliteal ven DVT tanısından sonra v. Cava filtresi implante edilmiş.
    Heparin tedavisi almış ve şu anda kumadin kullanıyor. Son INR; 2.04 bulunmuş.
    Havaalanına transferinde stabil idi.
    Hasta uçakta, sedye ile transfer edildi. Uçuşun ilk bacağında her hangi bir sorun yaşanmadı.
    Hasta uçuş boyunca yatar pozisyonda ve ayakları elevasyonda idi.
    Hasta İstanbul Atatürk havalimanında stabil idi. SpO2’si en düşük % 95 tesbit edildi. Her
    hangi bir girişim yapılmadı.
    Uçuşun ikinci bacağında SpO2 stabil seyretti ve destek gerekmedi. Hasta yatar pozisyonda ve
    uçuş boyunca bacakları elevasyonda idi.
    Havalimanında sedyeli yardım ekibi beklemekteydi ve hasta doğrudan ambulansa alındı.
    Hasta tüm dökümanları ve uçuş sırasındaki medikal durumu belirten rapor ile birlikte
    ambulansa teslim edildi.
    Transfer sırasında beklenmedik bir problem gelişmedi.

Case 3


Name: HH
DOB: 20.06.1941
Tanı: KOAH Akut Alevlenme + pnömoni(?), Koroner Arter Hastalığı, yüksek ventriküler
hızlı AF, Hipertansiyon
XX tarihinde hastaneye yatırılmış ve 10 gündür yukardaki problemleri nedeniyle tedavi
görmüş. Hasta ara ara ortaya çıkan taşiaritmiler ve solunum sıkıntısıyla stabilize olmuş.
Transport Report
Hasta XX tarihinde saat 10:30’da hastanede görüldü. Muayenede; yorgun görünüyordu
(tedavisi sırasında yaklaşık 15 kilo verdiğini ifade etti). TA: 132/75 mmHg, Nb: 114/dak-
düzensiz, akciğer oskültasyonunda ronküsler, SpO2: %94 (4lt/dak O2 akımda). Hasta transfer
için hazırlandı ve saat 11:30’da ambulansa alındı.
Havalimanına transferi sırasında hasta stabil idi.
Uçuşun ilk bacağında büyük bir problem yaşanmadı. Uçuş seviyesinde SpO2 %90 bulundu ve
2lt/dak O2 desteği sağlandı. Bir süre sonra SpO2 %96’ya ulaştı ve O2 desteği sonlandırıldı.
Hasta İstanbul Atatürk havalimanında stabil idi. SpO2’si bu dönemde en düşük %91 oldu. Her
hangi bir girişim yapılmadı.
Uçuşun ikinci bacağında, SpO2 %93 olarak başladı fakat uçuş seviyesine ulaşıldığında % 91
ve daha altına düşüş gösterdi ve Yyaklaşık 15 dakika O2 desteğine başlandı (SpO2 %96’ya
yükseldi). Hasta çok az miktarda yemek yedi. 15 dakika sonra SpO2 % 88’e düştü ve bunun
üzerine son yaklaşmaya kadar tüm uçuş boyunca hastaya 2lt/dak O2 desteği verildi. Hastanın
SpO2’si %96-97 dolaylarında stabil seyretti.
Havalimanında tekerlekli sandalye mevcut idi ve hasta doruca ambulansa alındı.
Hasta tüm medikal dökümanları ve transfer sırasındaki durumu gösteren bir medikal not ile,
ambulans ekibine teslim edildi.
Hasta transfer görevi, her hangi beklenmedik bir problem gelişmeden tamamlandı.

Case 2


  • Name: TG
    DOB: 9 y.o. / M
    Diagnosis: Behcet Disease + DVT
    Admitted to Hospital on XX and treated fort he above problems since almost 30 days. The
    patient was stabilised with some interventions. As the patient’s family were expatriates in
    Europe and their holiday situations were limited, they wanted their chilren to be transferred
    for permanent treatment.
    Transport Report
    The patient was seen on XX. On the examination; He looked tired. His legs were swollen. He
    was having troubles and pain on walking, although he could walk short distances. He has v.
    Cava filter implantation 20 days ago after diagnosed as right popliteal DVT. He had heparin
    and currently he was on coumadine. Last INR: 2.04 found.
    During his ground transport to the airport, he was stable.
    The patient was transferred on stretcher in the plane. There was no major problem at the first
    leg of the flight. The patient kept laid down and legs were kept elevated.
    The patient was stable at İstanbul Atatürk Airport. His SpO2 was 95 lowest during this period.
    No intervention was done.
    At the second leg of the flight, the SpO2 was stable and no support was needed. The patient
    was in lying position, legs elevated during the flight.
    There was stretcher assistance on arrival and the patient was taken to Ambulance directly.
    The patient was given over to the Ambulance Team, with all his medical documents and a
    small note of medical situation during the flight.
    The transport eascort mission was completed without any unexpected medical problem.

  • Name: HH
    DOB: 20.06.1941
    Diagnosis: COPD acute attack + pneumonia(?), Coronary arterial disease, AF with high
    ventricular rate, Hypertansion
    Admitted to Hospital on XX and treated fort he above problems since 10 days. The patient
    was stabilised with occasional tachyaritmias and shortness of breathing.
    Transport Report
    The patient was seen at the hospital on XX at 10:30. On the examination; He looked tired (he
    explained that he was lost about 15 kg.s during his treatment). BP: 132/75 mmHg, Pulse:
    114/min-irregular, Ronchuses on lung oescultation, SpO2:94 (with 4lt/min O2 flow). He’s
    prepared for the transport and collected by Ambulance at 11:30.
    During his transport to the airport, he was stable.
    There was no major problem at the first leg of the flight. As the SpO2 was found 90 at flight
    level, he was given 2lt/min O2. As the SpO2 reached to 96, O2 treatment stopped then.
    The patient was stable at İstanbul Atatürk Airport. His SpO2 was 91 lowest during this period.
    No intervention was done.
    At the second leg of the flight, the SpO2 started with 93 but following the reach to the flight
    level, droped down to 91 and O2 was given for 15 minutes (SpO2:96). The patient had very
    little amount of meal. 15 minutes later the SpO2 was 88 and he was given 2lt/min O2 during
    the whole flight until the last approach of the flight. His SpO2 was stable at 96-97 during this
    period.
    There was wheelchair assistance on arrival and the patient was taken to Ambulance directly.
    The patient was given over to the Ambulance Team, with all his medical documents and a
    small note of medical situation during the flight.
    The transport eascort mission was completed without any unexpected medical problem.
